Дополнительные аспекты: коморбидность, демографические признаки



 

В заключение следует выделить еще некоторые эмпирические данные, которые хотя и не релевантны для этиологии непосредственно, но все же содержат вполне достойную внимания информацию о возможных фоновых условиях депрессивного развития. Как явствует из ряда эпидемиологических исследований по вопросам коморбидности, повышенная вероятность возникновения депрессий может быть следствием тревожных расстройств; обратная связь наблюдается значительно реже (Wacker, 1995). Ввиду того значения, которое в теоретических дискуссиях и терапевтической практике издавна придавалось последовательности тревога—депрессия, эти данные вызывают повышенный интерес (ср. Wolpe, 1971; 1990; и др.). Другую релевантную фоновую информацию дают демографические факторы риска, разработанные в соответствующих эпидемиологических исследованиях. То, что болезненность депрессивными расстройствами у женщин существенно выше, чем у мужчин, по понятным причинам порождает много важных этиологических вопросов (Nolen-Hoeksema, 1990); то же самое справедливо и для различий в болезненности расстройствами в зависимости от возраста, например доказано, что у молодых людей частота депрессивных расстройств значительно возрастает, причем частота депрессий возрастает после пубертатного периода (Wittchen, 1994; Harrington, 1993). Объяснения таким данным, скорее всего, будут найдены в рамках интегративных моделей депрессии.

 

Заключение

 

Кратко резюмируем все вышесказанное. Очевидно, что на различных уровнях есть факторы, чья роль в генезе депрессии как потенциальных факторов влияния — стрессовых или диспозиционных факторов воздействия — сегодня сравнительно хорошо доказана. Вместе с этим очевидно и то, что этиологическое исследование депрессий сегодня еще только начинает развиваться. Процессы, происходящие в биологической, психосоциальной и психологической плоскостях, оказались значительно более сложными, чем это предполагалось в первоначальных гипотезах 70-х и 80-х гг.; дифференцированный концептуальный и эмпирический анализ этих процессов, и прежде всего взаимосвязей между ними, только начинается. А если еще учесть, что многие проблемы специфики депрессивного заболевания также остаются нерешенными, например значительная часть приведенных выше факторов влияния являются релевантными не только в продромальной фазе депрессии, но и других психических расстройств, то становится ясным, сколько еще предстоит сделать в этом отношении в будущем.

Тот факт, что к совершенному пониманию многомерной структуры условий возникновения депрессивных расстройств можно прийти лишь постепенно, не нуждается в дополнительном пояснении. Несомненно, будущие исследования должны сконцентрироваться на систематической эмпирической проверке интегративных моделей. Как подчеркивалось выше, гипотезы, выдвинутые в контексте общей концепции диатеза-стресса, заслуживают серьезного внимания; их необходимо развивать и далее, учитывая и другие специфические паттерны взаимосвязи между определенными факторами влияния. Разработка таких констелляций риска, как промежуточная цель на длинном пути к созданию крупных биопсихосоциальных моделей депрессии представляет собой интерес не только с точки зрения фундаментальной науки, но и с точки зрения прикладной клинической сферы.
